What a 5-Lug Adapter Does
A 5-lug adapter bolts to the vehicle hub and provides a different bolt pattern for the wheel.
Example:
Vehicle side: 5x4.5
Wheel side: 5x5
Adapter: 5x4.5 to 5x5
This allows a 5x5 wheel to be mounted on a 5x4.5 vehicle, assuming all other measurements are correct.

The first pattern should match your vehicle. The second pattern should match your wheel.
